EXICITING OPPORTUNITY TO FORM PART OF MASONIC CARE TASMANIA amp; RESPECT AGED CARE HOME CARE BUSINESS -Let’s be the change to shape the ‘Age Care at Home’ future.

We are seeking a highly skilled and compassionate Registered Nurse to join our Home Care community team. You will form part of the team to perform clinical assessments, delivering clinical care, provide clinical guidance to case managers and case conference facilitation to ensure the individualised, personalised care to our clients so they can continue to live in their home and focus on their quality of life. Each day will be different and flexible with the geographic region covering both Northern and Southern Tasmania.

Responsibilities include:

  • Provides personalised client care in the comfort of their homes including but not limited to, wound care, medication management and disease management.
  • Conduct thorough clinical assessments and develop comprehensive care plans based on individual needs.
  • Coordinate with local healthcare networks to ensure continuity of care and quality clinical case management for example, Primary health and multi disciplinary teams.
  • Collaboration with all areas across Home Care including case managers, field, scheduling and management.
  • Initiate and facilitate clinical case conferences to monitor and navigate clinical and operational solutions to maintain client health and wellbeing.
  • Be accountable for maintaining the high clinical standards aligned with compliance, governance and Aged Care Standards
  • Document client care accurately and timely.

Requirements

What you can bring to the role:

  • Current AHPRA registration as a Registered Nurse
  • Ability to conduct thorough clinical assessments, develop comprehensive care plans and clinical skills.
  • Self-starter with the ability to work independently and as part of a team.
  • A genuine passion for caring for the elderly.
  • Excellent communication, interpersonal and written skills
  • Strong organisational skills and ability to manage multiple cases simultaneously.
  • Ability to travel between Launceston, Hobart and surrounding areas.
  • Covid 19 vaccination
  • Previous experience in age care is desirable.
