Job Description
Fronditha Care is a longstanding Aged Care Provider committed to delivering services to Australian elders of Culturally and Linguistically Diverse Backgrounds. We focus on providing unique services both in residential and community care for elderly people of Greek speaking background in Victoria and of multilingual background in New South Wales.
An opportunity has arisen at the Thornbury Residential Facility, for an energetic self starter, who is a highly motivated and experienced individual, to join the site as a Clinical Care Coordinator (CCC) on a Full Time basis. The successful candidate will be someone who is resident focused and committed to quality care with strong people skills and a keen interest to deliver results.
Requirements
The successful candidate will have:
- A current registration certificate for a Registered Nurse.
- Knowledge and understanding of the physiology of the ageing process.
- Excellent skills in assessing, documenting and implementing resident’s care planning objectives.
- Sound Knowledge of the Accreditation Process.
- Sound knowledge of Aged Care Funding processes
- Ability to work in a team setting, both as a participant and a leader.
- Well developed problem solving, interpersonal, written and communication skills.
- Greek language skills are desirable, but not essential.
The CCC is fully supported by a strong Administration team and guidance provided by the Residential Care Manager.
The successful applicant will also be required to undergo a current police check
