The Position
Melba is seeking a Personal Outcome Measures (POMS) Planner to complete interviews with people supported, their natural supporters and relevant Melba staff to identify more specific and focused ambitions in line with the person’s broader NDIS goals and supports purchased from Melba. You will develop high quality accessible individualised plans to reflect what is identified as important to and for each person purchasing services from Melba through the completion of POMS interviews.

You are positive, energetic, and committed to achieving results, focussing on supporting people to achieve their personal outcomes by using your expertise in person centred planning. You must be highly flexible and adaptable and can demonstrate strong interpersonal and communication skills by building collaborative relationships with stakeholders and demonstrate a commitment to service excellence.
You’ll have:
- Previous experience working as a Team Leader/Supervisor of direct support workers in the Disability Sector
- Previous experience in the delivery of person-centred planning with an active support focus
- Working knowledge of the Victorian Disability Act 2006 and the NDIS Act 2-13
- Understanding of the philosophy and intent of the National Disability Insurance Scheme and the function of the NDIS Quality and Safeguarding Commission
- High level facilitation and interviewing skills
- High level Communication- written, verbal supported by your strong active listening skills
- High level organisational and time management skills with a proven ability to prioritise, meet deadlines and work autonomously
- An Australian drivers licence
Successful candidates will be required to obtain a NDIS Worker Screening Check and Working with Children Check.
