About the Role

  • Part Time, 2-3 days per week
  • CSD Level 2 from $63,670 - $68,264 p.a (pro rata) depending on skills and experience + Super + Salary Packaging
  • Maximum Term Contract until 30 June 2022
  • Moonee Ponds Hume, and Moreland The office is located in Broadmeadows.

The Integrated Chronic Care service is commissioned by the North Western Melbourne Primary Health Network (NWMPHN) to be delivered by Neami National in the Hume, Moreland and Moonee Valley council regions.

NWMPHN and Neami National are seeking to achieve the following outcomes with this service:

Individuals experiencing severe and persistent mental illnesses who can be or are being appropriately managed in a primary care setting:

  • Are supported to achieve improved outcomes, including better self-management, for their chronic conditions;
  • Are supported to access relevant to physical and mental health support

Core duties

  • Use the principles of Peer Work to deliver goal directed care planning whilst ensuring that the individual is included in the decisions of treating practitioners and teams
  • Provide support that builds the individual capacity to become a more active participant in their own care
  • Use narrative of personal recovery to support better physical health and mental health outcomes
  • Deliver care co-ordination informed by Peer Work principles to facilitate access to services in the community
  • Facilitate groups designed to support physical health and mental health outcomes
  • Work in close collaboration with the ICC Registered Nurse

What will you bring to the role?

  • Lived experience of mental illness and demonstrated experience of recovery
  • Experience of the public or private mental health system
  • Demonstrated experience in facilitating groups, including the ability to prioritise different needs within a group
  • Draw upon your understanding and belief in strength based, recovery orientated models of service
  • A clear ability to work within and to develop strong collaborative relationships
  • Extensive experience and the ability to engage consumers and carers in a positive manner
  • The ability to set and maintain clear professional boundaries
  • A commitment to consumers’ rights and to consumers’ active participation in planning their service
  • Excellent written and verbal communication skills
  • Knowledge of the recovery model
  • Computer skills

Mandatory requirements:

  • Current Working with Children Check
  • Australian Working Rights
  • Drivers licence
  • Access to a registered and insured car
  • Police check (refer to PD) disclosable outcomes considered
  • A Disability Worker Exclusion Scheme (DWES) Check will be undertaken prior to commencement
