About the Role

  • Full time
  • Maximum term contract until 31 December 2021
  • CSD 3: $73,434 - $79101 p.a, pro rata (depending on skills and experience) + Super +Sal Packaging + Benefits
  • Location: East Victoria park (option to work from Joondalup and Rockingham)

The Intake Assessment Worker will support new consumers of the identified cohort with NDIS access applications working in collaboration with the clinical team. The task will involve onboarding new clients, support with evidence gathering, submitting a NDIS access request form and supports with NDIS planning meeting. The NDIS transition worker is expected to work across Perth Metro Region and support NDIS applications for consumers in Bunbury and Albany (remotely).

Core duties

  • Receive Requests, Assess and Provide Appropriate Support and Advice
  • Accepting referrals from the services seeking clinical evidence to complete the access process, undertaking a pre-screen to determine likely eligibility for the NDIS and offering support to complete the access process, as required.
  • Collecting and collating all necessary evidence and completing the Access Request Form with the consumer and their supporters, in collaboration with the person’s (past and present) treating health professional,
  • Assisting the person to submit their Access Request Form to the NDIA and acting as a contact person for NDIA queries, at the request of the person,
  • Follow-up with the NDIA on the status of the consumer’s access request (as required) including monitoring of timelines

What will you bring to the role?

  • Relevant experience in an intake role
  • Ability and willingness to work effectively as part of a team, to meet program goals and objectives
  • Outstanding IT and phone handling skills
  • Demonstrated experience in managing competing priorities
  • Knowledge of complex mental health and drug and alcohol issues
  • Knowledge of local services particularly relevant to designated program area
  • The ability to form and maintain strong relationships with key stakeholders
  • Highly developed written and verbal communication skills
  • Be able to work with individuals from diverse and unique backgrounds
  • Be an honest and transparent individual
  • Knowledge of the recovery model
  • Qualifications in mental health

Mandatory requirements:

  • Working with Children checks (are required before commencement of work)
  • Australian Working Rights
  • Drivers licence
  • Police check (refer to PD) disclosable outcomes considered
