Job Description

Planned Parenthood of Greater New York is seeking aCase Managerto join our Project Street Beat team. This is a full-time position located in New York City at the Project Street Beat offices in Brooklyn and the Bronx.
The Case Manager provides comprehensive case management for individuals at risk of or living with HIV. May include advocacy, risk reduction education and counseling, and providing emotional support to individuals engaging in or at risk of engaging in sexual and substance use behaviors, including sex work, intravenous drug use and other behaviors that increase risk of harm. Facilitate regular support groups for consumers. May involve field work including home visits and accompaniments as needed.

What the Case Manager does:
  • Program and Office Support
  • Determines individual’s eligibility for case management and other PSB services;
  • Responsible for conducting and documenting PSB intake, risk assessments and reassessments, service plans and ongoing services;
  • Provides emergency concrete services including but not limited to food, transportation and accompaniments to necessary services;
  • Intervenes with clients by educating them about the risk of HIV/AIDS, the need to protect themselves through the use of various risk reduction strategies, and provides them tools and information on proper usage;
  • Provides ongoing case management including; risk reduction counseling regarding sexual and substance use, linkages to medical, social and supportive services, referrals for housing and related services, follow-up/reengagement, accompaniments and home visits;
  • Facilitates support groups and group-level interventions;
  • Participate in regular outreach and recruitment events as requested.
  • Administrative Team Coordination
  • Accepts individual and joint responsibility for and participates in departmental efforts to comply with all organizational and quality assurance guidelines, and all applicable government regulations;
  • Works effectively with volunteers, PSB Health Promoters, trainees and temp agency personnel.
  • Budgetary/Statistical Support
  • Accountable to meet all policies and procedures for service delivery and documentation;
  • Accepts individual responsibility for program spending (e.g, program supplies, meals, etc.), distribution of incentives and documentation, according to the spending plan and PSB Incentive Distribution Policy.
  • Customer Service Responsibilities
  • Arranges for the provision of health services for clients either through the program's Mobile Health Center, other PPGNY health centers, or through referral to other providers.
Minimum Qualifications:
  • A demonstrated commitment to PPGNY’s mission related to bodily autonomy, health equity, and gender and racial justice
  • Bachelor’s degree or CASAC with at least 1 year in case management services.
  • You have a demonstrated understanding of the role that racial inequity plays in our society and its impact on healthcare systems and institutions, and consistently integrate your deep understanding of key racial equity concepts into work projects and interactions by addressing structural implications and disproportionate impacts of policies, activities, and decisions on race, class and other group identities within the context of job responsibilities and projects.
  • Sensitivity to cross cultural issues
  • Familiarity with social services.
  • Familiarity and comfort with harm reduction model.
  • Strong interpersonal skills.
  • Familiarity with PSB centered communities
  • Able to maintain confidentiality and HIPAA compliance.
Preferred Qualifications:
  • Experience working with issues such as homelessness, criminal justice system involvement, HIV/AIDS
