Job Description

Fronditha Care is a longstanding independent not-for-profit Aged Care Provider that focuses on providing unique, culturally and linguistically diverse services to a wide range of consumers within a multicultural environment.

Our Lower Templestowe residential facility is currently seeking a motivated and experienced Registered Nurse to join their team, in a Full Time capacity as a Clinical Care Coordinator (CCC). The successful candidate will be someone who is resident-focused and committed to quality care with strong people skills and keen interest to deliver results.

This could be an exciting opportunity for an experienced Registered Nurse keen to take a step into a more leadership role.

The role:

As the CCC, you will be responsible for the efficient and effective delivery of person-centered care and clinical expertise. In collaboration with the Residential Care Manager you will be:

  • Required to lead and support the team to deliver optimal results to our residents.
  • Coordinating care planning development and review to ensure all care requirements are consultative and effective.
  • Preparing of accurate and timely ACFI documentation.
  • Driving continuous improvement activities relevant to clinical care.
  • Leading all staff members responsible for the delivery of care to residents independence.

Requirements

The successful candidate will have:

  • A current Nursing Registration with AHPRA as a Registered Nurse.
  • Experience in the Aged Care Sector (preferred)
  • Strong management, communication and interpersonal skills
  • Excellent skills in assessing, documenting and implementing resident’s care planning objectives
  • Sound Knowledge of the Accreditation Process.
  • Sound knowledge of Aged Care Funding processes.
  • Ability to work in a team setting, both as a participant and a leader
  • Current police clearance.
