About Neami National
Neami National is a community mental health service supporting people living with mental illness to improve their health, live independently and pursue a life based on their own strengths, values and goals.
About your new role
The Supported Transition and Engagement Program (STEP) is a post crisis support service for people recently housed. It will provide personalised support to meet the needs of the individual or families, in response to the circumstances of each person, their experiences and choice.
The specific objective of this program is to deliver person-centred wrap around support to assist people to remain housed, and more importantly to break the cycle of homeless. The program will strive to achieve this through the provision of culturally appropriate health, mental health and wellbeing services; building family, community and cultural connections; supporting the development of daily living and self-management skills; and promoting engagement in activities such as social groups, education and/or employment.
The Service Manager is responsible for the establishment, ongoing management, and further development of the Program. The Service Manager will work collaboratively with partners to improve and develop the overall service response to people leaving homelessness and living with a mental illness within our community. The Manager will also undertake a key role in service development, including ongoing evaluation and review of service outcomes.
