Job Description

An exciting opportunity has become available for a full-time Registered Nurse for a Clinical Care Coordinator position at our Lyrebird Village home in Drouin, Victoria.

The Clinical Care Coordinator is responsible to plan, allocate and delegate duties and facilitate the delivery of care according to documented policies and procedures. The Clinical Care Coordinator will contribute to the effective administration of human, material and fiscal resource management according to budgetary requirements.

Requirements

What you can bring to the role:

  • Bachelor of Nursing
  • Current Australian Health Practitioner Regulation Agency (AHPRA) registration
  • Nursing experience within a residential aged care facility
  • Experience of working cooperatively with multi-disciplinary care teams
  • Strong clinical assessment, documentation and practice skills
  • Report to and work alongside the Facility Manager
  • Sound clinical knowledge and contemporary practice.
  • A professional attitude and presentation.
  • Highly developed communication and relationship management skills with the ability to relate to the elderly.
  • Exceptional problem-solving skills.
  • Demonstrated organisational skills, with accuracy and attention to detail.
  • Provide leadership support to the clinical and support staff
  • Proficient computer and information management skills, including ability to use Microsoft and clinical software systems

Desirable:

  • Experience in a similar clinical care role
  • An understanding of the aged care accreditation process
  • Working knowledge of quality improvement programs
  • Current Driver’s License.
