Job Description

We are looking for a Support Coordinator to join our Support Coordination team located in our Ballarat office on a permanent full time basis. This is a Hybrid Working position with flexibility to work from home.

Our support coordination services assist our supported people with everything to do with the NDIS, from accessing NDIS funding all the way through to receiving the right supports for them.

The Role

Responsibilities Include:

  • Maintain up to date knowledge and relationships with service providers
  • Help participants connect to NDIS and other supports (funded, mainstream, community and informal)
  • Build a participant’s capacity and capability to understand their NDIS Plan, navigate the NDIS and make their own decisions
  • Broker supports and services in line with a participant’s wishes and their plan budget
  • Support participants to trial options, design a support approach and negotiate service agreement in line with their preferences
  • Monitor plan budgets and support effectiveness
  • Engage in crisis planning, prevention mitigation and action as appropriate

About You

  • Previous experience in support coordination, local area coordination, social work or case management
  • You like to think outside the box and want to work for an organisation that values creativity, passion and fun!
