About Neami National
Neami National is a community mental health service supporting people living with mental illness to improve their health, live independently and pursue a life based on their own strengths, values and goals.
We have been providing mental health support for over 30 years. We were founded on an alliance between professionals, individuals and their families. Collaborative recovery remains at the heart of who we are today.
About your new role
The Wellness Connect Commonwealth Psychosocial Support Program provides short-term, flexible and recovery-orientated psychosocial support focused on capacity building and connection.
The Service Navigator is an additional service offered as an enhancement to Wellness Connect. The Service Navigator assists with identifying appropriate services tailored to the consumer’s individual needs and goals to support their recovery. The Service Navigator can offer more service choices and options and links consumers through supported referrals. The Service Navigator takes a collaborative approach, working with the consumer, their carer, and support worker to identify support needs and gaps and has a capacity and skill building focus, fostering independence and confidence building. The Service Navigator will work closely with consumers and consortium staff of the Wellness Connect Program.
