The Position:
The Case Manager is a full time position based in Sydney, New South Wales. The successful candidate will be responsible for the initial provision and case management of Home Care Packages for clients. The position is pivotal to directing client care covering appropriate intake and assessment, care planning, community engagement and documentation, monitoring and review, fund management and advocacy as required.
A dynamic new role for our Sydney team, working in close partnership with the Chief of Home Care. You will form part of a collaborative, agile team with a refreshing approach and culture, as we strive to support our clients to ‘live life as they choose’.
